There is no set format but should cover the following An Introduction giving the purpose, scope and structure Details about the project or program, including the theory of change and logical framwewqork Approach to evaluation Monitoring approach, including indicators and reporting Data collection methods Responsibilities of who is doing what A bar chart with all the activities over time can also be useful A graphical diagram of the framework is also very useful Developing the framework and plan should be one of the first steps to be taken when starting a new program or project.
