DEADLINE Every Way to Die in Tomb Raider 1

For the achievement "Deadline: find all 36 ways to die" I show every death that counts as a numbered entry, along with some bonus variant deaths which do not count on their own. Death by lava and embers count as the same death as death by fire. The statue centaurs and the centaurs that hatch from pods count as the same death. Winged mutants and ground mutants count as two separate deaths. You cannot let the winged mutants kill you by pushing you off ledges, they have to kill you by mauling. When I first attempted this achievement death by swimming rats did not count as death by rat. I am not sure if this is still the case or if it is the same for swimming crocodiles vs. walking crocodiles.
