Process maps with multiple tasks: Putting too many tasks on one process map makes it difficult to understand and execute for the process doers. To ensure easy understanding, a process length should be between 5 and 20 tasks. 4. Inaccurate standards for process mapping: Not paying attention to defining standards and methodologies leads to inconsistent documentation, which becomes difficult to understand and hampers the improvement initiative. 5. Documenting steps happening outside your organisations: These include the steps done by your customers or vendors, hence you should not be documented.
