Dragon Age Inquisition: Bug Glitch (Funny).
After using an AOE spell on you and your comrades on the bridge in the tutorial part, you lose all sfx and voice overs inside the game. Also, the NPC's during the cutscene glitches out and gets stuck in a T-Pose thus creating a progression break.
