ZABETAKIS’S THEORY OF CAUSATION Dr. Michael Zabetakis provides a fourth update of domino theory of accident causation to assist mine management. While similar to others, it also introduces a new concept that the direct cause is an unplanned release of energy and/or hazardous material. The notion that accidents are caused by the unplanned transfer, or release, of energy is currently a part of a number of theories of causation. Most accidents are actually caused by the unplanned or unwanted release of excessive amounts of energy (mechanical, electrical, chemical, thermal, ionizing radiation) or hazardous materials (such as carbon monoxide, carbon dioxide, hydrogen sulfide, methane & water). However, with few exceptions, these releases are in turn caused by unsafe acts & unsafe conditions. That is, an unsafe act or an unsafe condition may trigger the release of large amounts of energy or of hazardous materials which in turn causes the accident. While we often think hazardous acts and conditions as the basic causes of accidents, they are actually only symptoms of failure. The basic causes are usually traceable to poor management policies and decisions, and personal and environmental factors. In fact selection, training & placement of each employee and the purchase, inspection & maintenance of each piece of equipment, machines and all other supplies must be considered as important to a successful accident prevention program as are the maintenance of a safe & healthful environment & the establishment of adequate operating & emergency procedures. That is, an unsafe act or an unsafe condition may trigger the release of large amounts of energy or of hazardous materials which in turn causes the accident.
