OUT OF CONTROL - COLUMBIA EXPLOSION OF THE SPACE SHUTTLE / February 1st 2003 : Spaceship Columbia is destroyed when entering the atmosphere after a two week long mission. The causes : some damages on the thermal protection system of the spaceship provoked a structural default on the left wing that eventually created an explosion. Investigations revealed that the damages on the reinforced part of the wing were caused by a block of isolating froth that was detached from the exterior tank during take off.
