Due to this fear, they often discontinue their treatment in the middle. The term hysteroscopy means inserting a camera and light inside the uterus to examine whether the uterine lining is intact and normal or not. There should be no presence of any masses, polyps, or other abnormalities. And laparoscopy refers to inflating the abdomen with gas, inserting a camera and light to see the areas surrounding the uterus, ensuring there is no underlying cause for the woman's inability to conceive. There are three common causes for this condition. The first cause is adhesions. Adhesions are like tubes that are attached to the uterus. Their job is to travel and pick up the egg. However, they are not performing their task properly because they are stuck. Adhesions are mostly found in cases where the patient has previously undergone abdominal surgery, such as appendectomy or any open surgery in the abdomen or mouth, or has a history of injury. Most patients have a history of previous surgeries or surgeries related to appendectomy in girls. We also suspect the presence of adhesions in cases of endometriosis or chocolate cyst, where the disease affects the body and causes adhesion to occur in the intestines and omentum. The second aspect we look for in diagnostic hysteroscopy is whether the tubes are open or not. The third aspect we want to observe is whether endometriosis or chocolate cyst is visible, which may not show any symptoms in the patient. Additionally, we check if there is any inflammation or fluid accumulation in the tubes or if there are any signs of infection. The most common infection observed is some form of microbial infection. Most cases of tuberculosis are also manifested in this way. We look for the presence of adhesions and also examine if there is any small tubercle, which can be sampled for further testing and evaluation. If the treatment requires further intervention, such as closing the fallopian tubes filled with water, it is crucial to obtain the patient's consent. Therefore, we prioritize having a discussion with the patient first, especially since we already have an ultrasound indicating a potential possibility for success using this approach. Subsequently, we communicate with the husband and proceed with the necessary steps, considering the patient's unconscious state during the procedure. The patient may have concerns, particularly about the risks associated with being unconscious. To address this, we explain the diagnostic histolaparoscopy procedure, which involves using an endoscope to examine the patient while they are under anesthesia. It is important to note that we do not perform any unrelated surgeries during this process. To ensure the patient's fitness for unconsciousness and the procedure, we conduct kidney and liver tests, check for conditions like diabetes and blood pressure, perform a complete blood test, and determine the blood group.
