Splash Blast Panic - Hard, No Deaths
Top Ten Anime Battles achievement/trophy, which requires No Deaths or Continues on Hard difficulty. 150h+ grind to finally get this run. Starts at Stage 4, when the difficulty ramps up. The final boss's 2nd phase can not spawn, and there's no way to fix this short of uninstalling/reinstalling the game. I ran into this 6 times on route to this run. It may be related to minimizing the game or locking your PC, or suspending it on Xbox. I resorted to reinstalling after every run to ensure this was avoided.
