Type 1 diabetes (T1D) is an autoimmune disease that destroys the B cells of the pancreas (mostly in children and adolescents) within weeks to months, rendering insulin production impossible. However, it accounts for less than 5% of all diabetes cases and is based on the dysfunction of T cells of the immune system. Thus, the genetic predisposition to T1D is primarily explained by variations in genes of the immune system. In addition, there are several monogenic forms of type 2 diabetes (T2D), which together represent only 1-2% of all T2D cases. This primarily affects genes of transcription factors that act in B cells. For the classic polygenic form of T2D, which occurs after age 40, genome-wide association studies (GWAS) have identified more than 100 genes that act in all metabolic organs. This makes T2D a highly heterogeneous disease that requires personalized treatment based on the individual patient genotype. Despite extensive studies, only about 10% of the risk of developing T2D can be explained by genetic variations, such as SNPs. This suggests that the majority of T2D cases have an epigenetic cause. Epigenetics affects the "packaging" of genes within the cell nucleus with chromatin and determines whether a gene can be used or not. Epigenetics is highly dynamic (genetics, in contrast, is not), meaning it reacts very sensitively to lifestyle changes, such as the composition of our diet, our weight, and the level of physical activity. The strongest epigenetic effects are evident during the embryonic phase, such as in response to malnutrition or poor nutrition in utero. However, even after birth, there are many ways to modify our epigenome so that we are not at risk of developing T2D.
