Rosenberg explained that things had once been smooth sailing, but now he was stuck in the middle of a power struggle among the three Mafia families: the Leone, Forelli, and Sindacco crime families. With no one trusting anyone else to manage the casino, he was caught in a precarious situation. Any hit between the families could easily backfire on him, especially since the Forellis had learned that Johnny Sindacco was recovering from a coma at the hospital—a coma caused by Carl himself. To protect Ken, Carl offered to move Johnny to a safer location. Heading straight to the hospital, Carl looked for Johnny, but the receptionist informed him that someone had already picked him up in an ambulance. Realizing it was likely one of the mob families, Carl quickly took off in pursuit. After some searching, he located the right ambulance and decided to take control of it. To do so, he banged up the ambulance enough to force the Forelli members inside to bail out. Once they were out, Carl jumped into the driver's seat and sped toward the meat factory in Whitewood Estates. As he drove, Forelli backup started closing in, but Carl skillfully evaded them. He finally arrived at the abattoir, where he spotted two Sindacco thugs approaching with Johnny in a wheelchair.
