Ovarian cysts are fluid-filled sacs in the ovaries, often found during routine ultrasounds. Most are harmless and do not require surgery. Does Every Ovarian Cyst Need Surgery? Absolutely not! Surgery is not needed for all ovarian cysts. Most can be safely monitored depending on size, type, and risk factors. How Do Doctors Decide? Doctors consider age, menstrual status, family and personal history, and risk factors like endometriosis or smoking. Blood tests like CA-125 and sometimes HE4 are also used. What Role Does Ultrasound Play? Ultrasound shows the type and size of cysts. Transvaginal ultrasounds and Doppler imaging help check blood flow and detect potential complications. What Is O-RADS Scoring? O-RADS grades ovarian cysts from 1 to 5. Scores 1–2 indicate very low cancer risk and often do not need surgery. Complex or high-risk cysts may need closer evaluation. When Can Surgery Be Avoided? Surgery can be avoided if: The cyst is simple O-RADS score is 1 or 2 CA-125 is normal The patient has no symptoms Size is under 10 cm When Is Surgery Recommended? Surgery may be needed if: CA-125 is elevated The cyst is larger than 10 cm The patient has symptoms The cyst grows or doesn’t shrink Imaging shows a complex cyst Why Observation Works?
