Proximal Humerus Fracture Treatment Explained | Non-Surgical Care, Sling & Rehab Proximal humerus fractures (fractures of the upper arm bone near the shoulder) are common, especially in older adults and after falls. Many of these fractures can be successfully treated without surgery, particularly when they are minimally displaced or stable. In this video, we explain the non-surgical management approach, including immobilization, rehabilitation, and healing expectations. Non-Surgical Treatment Overview Non-operative treatment focuses on: Pain control Protection of the fracture Early, safe restoration of shoulder movement This approach is commonly used for minimally displaced proximal humerus fractures. 1. Immobilization Phase Purpose: Stabilize the fracture Reduce pain Allow early bone healing Method: Arm sling is most commonly used Sometimes combined with a swath (body wrap support) Duration: Usually 2–3 weeks, depending on fracture stability and patient factors Important Notes: Patients are often encouraged to maintain basic self-care activities such as dressing and hygiene Prolonged immobilization should be avoided when possible to reduce stiffness 2. Rehabilitation Phase Rehabilitation is essential to restore shoulder function and prevent long-term stiffness. Early Phase (1–2 weeks): Gentle pendulum exercises Passive or assisted range of motion as pain allows Progressive Phase: Gradual increase in shoulder mobility exercises Focus on restoring full range of motion Strengthening Phase: Begins once fracture healing is confirmed Targets rotator cuff and shoulder stabilizers Supervision: Physical therapy is highly recommended for safe progression 3. Monitoring and Follow-Up Regular X-rays are used to monitor fracture healing Clinical assessment ensures improving pain and mobility Watch for complications such as stiffness or malalignment When Surgery May Be Needed Non-surgical treatment may not be suitable if: Fracture is significantly displaced Multiple fracture fragments are present There is poor bone alignment Nonunion or delayed healing occurs Functional demands require faster or stronger recovery Prognosis Most minimally displaced proximal humerus fractures heal well with conservative treatment.
