Longer CVR & Subtitles | Delta Airlines Flight 191 | 2. August 1985 Special thanks to @elsopaipilla315 ! Date and Time: Date: Friday, August 2, 1985 Time: 18:05 CDT Aircraft Information: Type: Lockheed L-1011-385-1 TriStar Owner/Operator: Delta Air Lines Registration: N726DA Year of Manufacture: 1979 Flight Details: Flight Number: DAL 191 Flight Nature: Scheduled Passenger Flight Departure Airport: Fort Lauderdale–Hollywood Int'l Airport, FL (FLL/KFLL) Destination Airport: Los Angeles Int'l Airport, CA (LAX/KLAX) Stopover/Diversion: Dallas/Fort Worth Int'l Airport, TX (DFW/KDFW) Location of Accident: Dallas/Fort Worth Int'l Airport, north of Runway 17L Accident and Casualties: Fatalities: 137 (126 passengers, 8 crew, 3 ground) Injuries: 27 (survivors) Occupants: 163 (152 passengers, 11 crew) Aircraft Damage: Destroyed (impact forces and fire) Phase of Flight: Final Approach Investigating Agency: National Transportation Safety Board (NTSB) Accident Narrative: Delta Flight 191 was a scheduled flight from Fort Lauderdale to Los Angeles with a stop in Dallas. During approach to DFW in a severe thunderstorm, the L-1011 encountered a microburst-induced wind shear. The aircraft abruptly lost altitude, struck a car on a highway north of the runway, and crashed into two water tanks. The impact and post-crash fire destroyed the aircraft. Probable Cause: The NTSB determined the probable causes of the accident were: Failure to avoid a microburst-induced wind shear during approach. Lack of real-time wind shear detection systems at DFW. Continued approach into known convective weather despite pilot reports of severe turbulence. Contributing Factors: Inadequate FAA/ATC guidelines for wind shear avoidance (1985 standards). Delta’s training program did not fully address wind shear recovery techniques. Limited cockpit weather radar capability to detect microbursts. This accident revolutionized aviation safety by accelerating the deployment of Doppler weather radar (TDWR) and mandatory wind shear training for pilots.
