Air India 171, a Boeing 787 Dreamliner crashed on takeoff after the fuel cutoff switches were transitioned to "cutoff." One pilot asked "Why did you cutoff?" The other pilot responded "I did not cutoff." The engines could not be re-started before the plane crashed into a medical hostel complex. 242 of the 242 passengers were killed and the preliminary report says 19 on the ground.
