Severe mitral stenosis - notice the loud S1 and loss of the gap between S2 and the opening snap. As the patient is in a state of tachycardia, it is difficult to comment on other features of severe MS - however, upon resolution of the fast-run of SVT, the diastolic murmur was loudest and prolonged in the mitral region, best heard with the bell in the left lateral position in expiration.
