As the demographic changes are happening in the population, there is an increasing trend of esophageal cancer migrating towards the stomach, with more cancers being adenocarcinomas of the OG junction, rather than what used to be commonly squamous cell carcinoma of the upper esophagus. Adenocarcinoma is seen in younger population, particularly around the OG junction in obese patients, patients who are smokers and alcoholic. When you talk about upper GI surgeries, particularly esophageal and gastric cancers, most of the patients in Chennai, particularly our local population, come at a very late stage. Nearly uh two-thirds of the patients are in stage three and four of disease. When they present to us in a locally advanced stage, We find that only a quarter of them come for effective curative resections or curative treatment. Most of them would have palliative treatment. In those patients where curative resection is contemplated, it's very, very important they get a very precise surgical treatment and precise surgery is done in institution with highly qualified people who do more advanced techniques, including a laparoscopic or robotic surgery. Robotic surgery and laparoscopic surgery, they have brought down the hospital stay and faster recovery after surgery.
