Today I am going to tell you about a case in which all the reports were normal, yet pregnancy was not happening; and finally the diagnosis was abdominal tuberculosis. When this patient came to me, initially I could not understand where the problem was. Everything was superficially normal. But when we went into depth and evaluated; a hidden reason came to light, which is often ignored. This patient came to me in 2022. At that time she was 34 years old. Her periods were regular Both fallopian tubes were open Her husband's semen analysis was also normal; sperm count, motility, morphology, and even DNA fragmentation was all fine Still pregnancy was not happening for two years. IUI had also failed We did 3 cycles of IUI first; but they were also unsuccessful. The diagnosis was: Unexplained infertility. But unexplained means "no reason can be seen from what we have seen so far", not "there is no reason". So I advised her to get a diagnostic hystero-laparoscopy done, and that is where we found out that she had abdominal tuberculosis (TB). This is a condition in which the uterus or ovaries do not show infection, but a silent TB infection is active in the abdominal cavity. How does TB impact fertility? Falopian tube dysfunction: The tubes remain open, but their primary function, that is to catch the ovulated egg, does not work. The size of an ovum is only 120 microns; the tube has to function actively to catch it. This function can be disturbed in TB. Endometrial damage: When genital TB occurs, the inner lining of the uterus (endometrium) can also be damaged. This damage extends to the basal layer and the endometrium becomes thin or completely destroyed. This causes periods to gradually become lighter or stop altogether. Management and recovery plan We immediately referred the patient to a physician where she underwent anti-TB therapy for 9 months. She came back to me after that. We tried a few cycles naturally and with IUI but I explained to her that functional failure is common in TB cases even when the fallopian tubes are open. Advanced regenerative options were also explained. If there was endometrial damage, we would have to consider options like Adhesiolysis via hysteroscopy PRP therapy and sometimes stem cell therapy from bone marrow But fortunately, her endometrium was good. So we decided to do IVF now. IVF cycle and final outcome We did ovum pick-up; 4 embryos were formed. The first embryo transfer failed But the second transfer was successful; And as a result they got a lovely healthy baby girl.
