The FRAIL-AF trial showed that warfarin may be safer than direct oral anticoagulants, at least in older patients with atrial fibrillation. This study aimed to provide evidence for transitioning atrial fibrillation patients from vitamin K antagonists, primarily warfarin, to direct oral anticoagulants (DOACs) like rivaroxaban, dabigatran, apixaban, and edoxaban, in anticipation of lower major bleeding incidents. However, the results took an unexpected turn. Switching from a vitamin K antagonist to a DOAC led to an increased risk of bleeding, with a hazard ratio of 1.69 and a highly significant P-value of 0.001. These findings suggest that such a transition, particularly in elderly patients, is not appropriate. The news is good at least for healthcare systems where the patient has to pay for the drug themselves, considering that DOACs are significantly more expensive than warfarin.
