COVID-19 vaccination consent form – BSL version

This is a British Sign Language (BSL) version of the COVID-19 vaccination consent form. The consent form asks for information of a personal nature which we tell you about in this BSL version. If you have a printed consent form please take it to your vaccination appointment. If you have not been given one, you may be offered one at your vaccination. You can ask to see this BSL version. Consent does not have to be in writing and therefore you may be asked to give your consent to vaccination either verbally or by non-verbal indication. If required, at your vaccination appointment you may ask for assistance from a member of NHS staff to help complete the form.
