Dr. Ebraheim’s educational animated video describes the condition of little leaguer's shoulder, the etiology, the symptoms, the diagnostic tests, and the treatment options. Is a stress fracture of the growth plate of the shoulder, it usually occurs due to throwing, it is considered to be a fracture, a Salter- Harris I. The overuse and repeated high load of torque, and a rapidly growing child causes micro-trauma to the physis. This injury is seen mostly in adolescence, pitchers and tennis players. The zone of hypertrophy of the growth plate is a weak zone, and that’s why it is affected. The overuse injury associated with pitching and is influenced by the quantity, intensity, and age of the player. Symptoms: Shoulder pain, worse with throwing, tenderness over the physis of the proximal humerus. Rule out subdeltoid bursa, that pain improves with rest. X-rays will show widened physis compared to the other side. Fragmentation and sclerosis of the lateral aspect of the proximal humerus in the physeal area. Treatment: Cessation of throwing for 2-3 months. Progressive throwing program. Rotator cuff strengthening once full painless range of motion is achieved. Complications: Premature growth arrest. Physeal fracture. Prevention: avoid overuse.
