It is the sequel to A Plague Tale: Innocence, and follows siblings continuing on with the story of Amicia and Hugo de Rune as they journey across Provence to find a cure for Hugo's condition. In 1349, after the events of the first game, siblings Amicia and Hugo, along with their mother Beatrice and her apprentice Lucas, seek refuge with an organization called the Order. They face attacks, causing Hugo's dangerous condition to worsen. Seeking a cure, Amicia and Hugo set out to find an island from Hugo's dreams. Along the way, they face challenges from soldiers and mercenaries. They eventually reach La Cuna, where they discover a pagan deity worshiped by the residents. As they uncover the history of a previous Macula carrier, they realize the Macula has its own plans for Hugo. They escape an ancient prison, reunite with Beatrice and Lucas, and confront a plot to use Hugo's power. Tragedy strikes as Beatrice is sacrificed, and Hugo, believing Amicia to be dead, succumbs to the Macula's influence, unleashing a devastating plague of rats.
