Its a very lucid video explaining the meaning of TMT test / Treadmill test. What does it means? Why we do it? What is the significance of positive TMT? Everything is explained simply in Hindi with a live TMT going on a patient. Regarding TMT, I want to add some reasonable points. TMT is called as Poor man's Angiography. Why it is called so? What is the mechanism of Positive TMT? Basically, positive TMT denotes that there is shortage of blood in myocardium at stress & that shortage is reflected through ST depression in TMT. Till now, we knew that, That shortage of blood occur due to significant blockages in Coronaries. But there is other mechanisms also, in which without significant coronary stenosis , ischaemia can occur in myocardium - 1. Micro-circulation defect, 2. Coronary spasm. When these mechanisms are implemented in causing myocardium Ischaemia, then TMT can be positive, but Angiography is mostly normal. So here we can understand many a times, what we called as false positive TMT, is actually myocardial ischaemia due to other mechanism(which is discussed above). So these causes of Angina should be medically dealt. Now we know that Why TMT is so important investigation. Many patients with micro-circulation pathology can have slow flow in Coronary angiography without any stenosis. These patients get classical Angina with Positive Tmt & this can also be treated medically. This is Often called as Syndrome X ( Angina with normal angiography). So TMT is a great tool which simply detects whether heart get enough blood or not. TMT is also useful in known CAD patients. Overall, TMT is a great tool & still it's underutilised. & due to its enormous utility, its use should be generous.
