To eliminate TB in 2035: -Must be able to diagnose active TB -Must identify latent TB or (LTBI) At first, Mycobacterium tuberculosis is acquired. Then cellular immunity will contain it The infection can then remain latent or cleared off The individual will be asymptomatic or non-infections But LTBI can become active Detection and treatment of LTBI will save the patient and the general populace Tuberculin Skin Test [TST] Interferon-Gamma Release Assays [IGRAS] Exclude an active TB Now wise to screen for HIV from day one Who should be screened? -Contacts of active PTB -IVD users -Homeless -Prisoners -Care workers in prisons/homeless shelters -Visitors to endemic regions -HIV positive -Immunosuppressed e.g. transplant/chemo cancers In many cases, the initial test is TST If TST positive: Confirm with IGRA Also r/o active TB with history/PE/X-ray If TST is negative, use IGRA to be sure IGRA [ Interferon Gamma Release Assay ] AND MANY MORE PIECES OF INFO,ENJOY LISTENING!
