Does HRT increase Risk of Stroke Blood Clots? The answer..it depends on the formulation and delivery method. So, while certain types of hormone therapy do increase risk. The good news is that transdermal bioidentical hormone replacement therapy does NOT increase risk of stoke, blood clots, deep vein thrombosis or thromboembolism. Bioidentical transdermal estrogen is often considered to have a lower risk of blood clotting and stroke compared to oral estrogen. This belief is based on several factors: Bypassing the Liver: Transdermal estrogen is absorbed through the skin and enters the bloodstream directly. This bypasses the liver and avoids the "first-pass metabolism" that occurs with oral estrogens. The first-pass effect is responsible for increasing certain clotting factors in the liver, which is a key risk factor for clotting and stroke associated with oral estrogens. Lower Doses: Transdermal estrogen can be effective at lower doses compared to oral forms, which may contribute to a lower risk profile. Different Metabolic Pathways: The metabolism of transdermal estrogen differs from oral estrogen, potentially leading to a different risk profile regarding clotting and stroke. Research Studies: There have been studies suggesting that transdermal estrogen may have a lower risk of thrombotic events (like blood clots) compared to oral estrogen. These studies often point to the different ways transdermal and oral estrogens are processed in the body. The EStrogen and THromboEmbolism Risk (ESTHER) Study: This study, published in the journal "Circulation" in 2007, investigated the thrombotic risk associated with oral and transdermal estrogen therapies in postmenopausal women. The ESTHER study found that transdermal estrogen was associated with a lower risk of venous thromboembolism (VTE) compared to oral estrogen. Specifically, the research indicated that the risk of VTE was significantly increased in women using oral estrogen but not in those using transdermal estrogen.
