Although he gets killed, he always manages to come back. Which is the case where he got killed by the cube only to be revived by it. And we saw him return in the form of Galvatron after being decapitated and used his chromosomes to gain control over his knew body. But did you notice this about his head in Transformers Age of Extinction? Its in somewhat good shape for someone who got an axe stabbed on the top of his head. Not only that but it looks more like his head from the 2007 movie. Than it did in Transformers Dark of the Moon. Which was a lot rustier and had a wider jawline. Which is funny because the concept art uses his head design from Dark of the Moon. As to why his head looked this way it could be that it was still able to regenerate and ultimately changed back to its original state from the 2007 film.
